Good question. I've wondered the same thing because I've never seen a buck have one spike two years in a row. That means either they get better, never visit my property again, or they die. I've also never seen a buck have a double main beam twice in a row. In my experience a lot of antler abnormalities are confined to the year following the initial injury and then get better. But really I only know what I've personally seen.

Antler deformities due to body injury can get better over time. Notice I said "can," not "will." Research on body-injury-induced antler abnormalities has shown everything for "gone the next year," to "gone over a number of years," to "permanent."
Now pedicle-injury-induced abnormalities are definitely permanent and get worse over time. Those can usually be identified when a buck grows an antler from the injured pedicle that forms a big single spike, or a cluster of spikes.
