Thoughts on what caused this

I'd say his testicles were compromised in a fence crossing accident. I've seen that cause constant antler growth year round a time or two before.

I'd say that's probably correct. Given his big body, skinny "doe" neck, and what looks to still be velvet, I'm thinking he's been robbed of his manhood at some point.

The first time I recall this same scenario my best friend called me and said on the last weekend of rifle season in about 07 or 08 "I have a screwed up buck in velvet still with huge bases and a bunch of spike points - what should I do" -" I said I can't shoot it you have to" all I heard was boooooom and long story short the local GW told us the go- nad scenario and I have read and seen multiple scenarios in the since years of the same outcome

When I was a boy, a fellow at our church shot an enormous racked 18pt with velvet, during gun season. Made quite a splash in the local area. The game warden who inspected it said it was basically neutered, which allowed its antlers to continually grow.
 
It's called a "cactus rack" and is caused by injury/loss/non-function of the testicles. The buck will grow the same set of antlers all his life. They will never mineralize to bone, and the velvet will never fall off. Sometimes extreme cold temperatures will freeze the antlers and they will die and fall off, which starts the process all over again.
