2-blade (like a "Rage" brand) mechanical broadhead hit is what it looks like.

Even though that deer appears to have survived ongoing,
I suspect he has some serious physical issues, which may include one lung.

Also notice how he's holding up his right leg near identically in both pics.

With compromised lungs and less ability to run as fast,
he becomes easier prey for coyotes & dogs,
even if he is otherwise able to survive ok.
 
Gonna go a different route than those above. From the pic I believe that to be a buck. Maybe an antler flesh wound from fighting. I cannot see how a deer would survive any pass through shot at that location.

There are videos and pictures of deer shot walking around that shouldn't be.

I immediately believed it to be a buck that had recently shed its antlers.
The wound could be from anything mentioned.
I did not assume it was a pass through wound.

What I find most instructive in trying to guess the cause is
that he appears to be holding up his right leg (not walking on it).
This could be mere happenstance, but rarely get 2 pics with a leg in same held up position like that.

Could have been a wound from more an angle, hit the leg bone mid-body. May or may not have caused ongoing lung damage. But animals can live on one lung.
 
Strangely, as soon as I put on my tinfoil hat, I realized it was a wound from a saber-toothed cat, which the TWRA recently secretly reintroduced into TN just to piss off deer hunters. While releasing rattlesnakes and black panthers they thought, "Let's give this species a try too."
